The following documents must be completed, signed and returned to Kids Unlimited in order to complete a student registration.
Please choose only ONE method of registration. If you have completed your registration online, do not submit the printed forms #1-7.

1. To register for the Kids Unlimited Summer Show Camp 2010, print forms #1 - #7
2. To register for the Kids Unlimited Dance Choirs, print form #12
3. If you have registered online for the Summer Show Camp, please print the Signature Page #8

After completing the required forms, please mail them to:
